If your note will require a name change to your official academic record (i.e. your transcript), Federal laws regarding identity theft prevention require that you submit a Name Change Request Form and include supporting documentation. The name change request is not required to submit a class note, but we strongly recommend you submit a request form as soon as possible to ensure your academic record is up-to-date.
